A RESOLUTION RESPECTFULLY REQUESTING HONORABLE EDCEL GREX LAGMAN,

GOVERNOR OF THE PROVINCE OF ALBAY TO ALLOCATE FUND FROM THE PROVINCIAL
GOVERNMENT FUNDS (PGA) OF THE BARANGAY ASSISTANCE PROGRAM FOR THE
PURCHASE OF P.E PIPE AT BARANGAY PAULBA, LIGAO CITY, ALBAY.

WHEREAS, RA 7160 otherwise known as Local Government Code of 1991 mandates

the Local officials to imitate projects benifit to its constituents that lead in preservation and
security of life and properties;

WHEREAS;water system is one of the priority project of the Barangay council of

Barangay Paulba,Ligao City. It help us a lot in terms of everyday life .

WHEREAS, water is very much important element in life in terms of providing safe
drinking to the constituents,use in everyday work and also for the animals as well.

WHEREAS, in order to reach the water to the respective houses ,we are needing a
pipe line to connect from the water source down to our constituents to be able to maximize
the resources of water.
 NOW THEREFORE, on motion of Barangay Kagawad Nerissa Racines, unanimously
seconded by the body be it,

RESOLVED AS IT IS HEREBY RESOLVED RESPECTFULLY REQUESTING HONORABLE EDCEL

GREX LAGMAN, GOVERNOR OF THE PROVINCE OF ALBAY TO ALLOCATE FUND FROM THE
PROVINCIAL GOVERNMENT FUNDS (PGA) OF THE BARANGAY ASSISTANCE PROGRAM FOR
THE PURCHASE OF P.E PIPE IN BARANGAY PAULBA, LIGAO CITY, ALBAY.

RESOLVED FURTHER, that copy of this resolution be furnished the Office of Hon.
Edcel Grex Lagman,Governor , Povince of Albay and other concern agencies.

Program (BAP) of the Province of albay for the Installation of Solar Streetlight of Barangay
Francia, Ligao City in the amount of ONE HUNDRED THOUSAND PESOS (P100,000.00).

Herein attached are the documentary requirements for your reference.

Furthermore, may we request LGU – Barangay Francia, Ligao City as the implementing
agency of the above-requested fund.

OMNIBUS SWORN STATEMENT

1. There is a Sangguniang Barangay Resolution endorsing the proposed project, Purchase and
Installation of Barangay Electrification (Streetlighting), and that the same part is part of the
duly approved Local Development Investment Program (LDIP) and Annual Investment
Program (AIP) of the Local Government Unit of Barangay Tambo , LIgao City, Albay;

2. The Local Government Unit of Barangay Tambo, Ligao City,

3. Albay has the capability to implement the project either by administration or contract;

4. The project of the Local Government Unit of Barangay Tambo, Ligao City, Albay is readily
implemented within the prescribed timeline and there are no issues related to the site/land
title, including informal settlers thereon, and right-of-way acquisitions;

5. The Local Government U it of Barangay Tambo, Ligao City, Albay has complied with the
standards prescribed by the appropriate agencies and has secured all the necessary
clearances or Certification from agencies before the implementation of the eligible project,
e.g. Environmental Clearance; and

6. The Local Government Unit of Barangay Tambo, Ligao City, Albay commits to fund the cost
of maintenance and repairs thereof, including payment of electricity, if any.
