    (c) Diploma / Degree Marksheets & Certificate (whichever is applicable)
    – All Semesters
    (d) Any other relevant marksheets / certificates pertaining to your
    Educational Qualification
    5. Documents pertaining to your official documents
    (a) Documentary Proof
    (b) Photocopy of PAN Card
    (c) Photocopy of AADHAR Card
    (d) Photograph in Professional Attire (Size – 4.5 X 3.5, front view, white
    background)
    6. Documents pertaining to your medical fitness & Covid-19 Vaccination
    (a) Your selection is subject to Pre-Employment Medical Examination by
    an Authorized Medical Practitioner. You need to submit a Medical
    Certificate determining your fitness.
    Kindly note the medical certificate should contain the following
    information:
    Name and signature of the doctor, registration number of the doctor,
    name of the clinic/hospital.
    (b) Covid-19 Vaccine Certificate
    Please upload your respective Covid-19 vaccine certificate
    (First dose & Second dose)
    7. Internship Letter
    8. Undertaking Form
    9. 12-page application form
