AFFIDAVIT

I, _____________________, of legal age, single/married,

Filipino, and with residence address at ___________________, after
having been duly sworn to in accordance with law, hereby depose
and state:

1. I am the Barangay Chairman of Barangay Libas, Merida,

Leyte. I am responsible for overseeing the peace and order in our
barangay as well as day-to-day operations in our office such as
receiving reports from residents and overseeing the patrolling of
barangay tanods.

2. During the period of 14-17 April 2022, no campaign

activities were observed by our officials nor did we receive any
reports from our residents on this matter.
A copy of the Barangay Blotter for 14-17 April 2022 is attached
hereto and made an integral part hereof as Annex “1”.

3. I am executing this affidavit to attest to the truth of the

foregoing statements and in support of the dismissal of the Petition
for Disqualification filed against Cong. Richard Gomez.
