To evaluate practice teacher’s performance during the actual teaching phase of the practice teaching program, the following criteria with their indicated
factor for evaluation shall be used:

CRITERIA RATING
A. PERSONAL AND SOCIAL QUALITIES (TEACHER’S PERSONALITY) – 10%
1. Is pleasing in general appearance.
2. Is responsible, dedicated to and enthusiastic in his work
3. Is resourceful and creative, and displays aesthetic sensibility
4. Gets along well with others even in an online learning environment
5. Follows a sound code of moral conduct and shows refinement in speech and manners

AVERAGE RATING (TOTAL/5)

B. ACADEMIC EQUIPMENT (TEACHER’S PREPARATION) – 10%

1. Understands educational objectives and provides ways of achieving them
2. Has a good understanding of curriculum requirements, of the pupils/students and of (virtual) community relations
3. Has an adequate knowledge and information and has subject matter content mastery
4. Shows evidence of logical thinking
5. Is clear and fluent in the medium of instruction

C. TEACHER ABILITY (TEACHING PRACTICES) - 20%

1. Shows evidence and thorough preparation of lesson plans and other instructional materials and devices
2. Uses effective and continuing motivation
3. Uses with skill a variety of teaching-learning procedures
4. Shows skill in the art of questioning and stimulates independent and critical thinking in pupils/students
5. Encourages creative activity and provides for maximum pupil/student participation

1. Uses appropriate self-made materials and other suitable instructional materials
2. Prepares activities that encourage pupils/students participation

5. Provides for useful extra-class activities
