NOTICE/AFFIDAVIT OF CANCELLATION OF
DEED OF CONDITIONAL SALE

I, ___________, of legal age, Filipino citizen, married and a resident of

____________________, after having been sworn to in accordance with law, hereby
deposes and states that:

1. I am the Vendor and the same person who executed a Deed of Conditional Sale
dated _________, docketed as Doc. no. __; Page No. __; Book No. II; Series of
2013, as reflected under notary public of Atty. ______ involving the parcel of land
identified as _____, located at _______;

2. Said Deed of Conditional Sale was entered with the Vendee, ____;

3. However, the term of paying the remaining balance of 80,000.00 within 24

months from date of execution of conditional sale was not fulfilled by the Vendee up
to the present;

4. I therefore wish to CANCEL, RESCIND and ANNUL said Deed of Conditional

Sale and declare the same Void AB INITIO for failure of the VENDEE to follow the
term and pay the full consideration of sale;

5. NOW, THEREFORE, in consideration of the foregoing premises, I, being the

Vendor, hereby unconditionally CANCEL, RESCIND, ANNUL and declare VOID AB
INITIO and without legal effect whatsoever the Deed of Conditional Sale I executed
with the Vendee on ______________;

6. Due to the cancellation of the deed of Conditional Sale, the buyer,

____________ right over the property has been extinguished. Therefore, it is
demanded that she vacate the property fifteen (15) from receipt of this notice;

7. I am executing this Notice/Affidavit of Cancellation of Deed of Conditional Sale in

order to attest to the truth of the foregoing and for whatever legal purpose this may
serve.

IN WITNESS WHEREOF, I have hereunto set my hands this 7 th day of April 2021 in
the City of Koronadal, South Cotabato, Philippines.
