me to its ability to recruit workers for foreign countries specifically in Dubai through an Internet
site.

That I, in full confidence applied to the person as a driver;

That is, the person mentioned promised me a job abroad for which in return I was

in the said house I was paid an amount of P4, 000.00 per month for my food and other expenses
for my temporary accommodation;

That, while I was temporarily staying in said house, he made me do as a driver of his car.
This is said to be part of my training as a care driver in Dubaii;

That, the person mentioned Ariel Sajulga failed to send me abroad Dubai I left for said
country as a Driver to work there on May 18, 2022 and returned on January 21, 2022 in Brgy
Lumbia Pagadian City Zamboanga Del Sur on the grounds that the family I was going to enter
Dubai backed out because they had already sold their car that I would going to drive there so that
my visa to Duai declined;

That is, the person mentioned failed and /or refused to return my money /papers despite
my repeated request;

That is, I have discovered that such person has no license or right to recruit and send
workers abroad;

That, until now the person mentioned has promised me that I will be able to continue to
leave for another country and he is demanding payment of P50,000.00 in exchange for my
alleged departure;

That, such person resides and/or may be notified in Brgy; Gatas Pagadian City
Zamboanga Del Sur;

That I have made this affidavit to sue Arfred Salabit and Aster Calma of illegal
recruitment and/or Human Trafficking;

That, I certify that I have not filed a complaint like this with the persons mentioned in
other agencies or branches of government herein.

I, Joseph Sabells, 24 years old, single, Filipino with residence at Brgy. Pob. Guipos
Zamboanga Del Sur do, hereby charge Mr. Robredo a PO4 in the police station of
Margosatubig,Zamboanga Del Sur with Murder committed as follows:

The family of the victim was reported to the Police Station of Guipos for the incident in
the Store of Maligaya in Purok 3B, Pob. Guipos, for what happen to their son who shoot by PO4
Robledo in the chest side for no reason. The result of the incident causing an instantaneous death
of the victim. The following information will be under investigation.

a) Affidavit of corroborating witnesses.

b) Enumerate other documentary evidence with a brief statement of the substance of
each.(to be attached)

In view thereof, it is respectfully prayed that after due investigation respondent PO4
Castaniares has be found guilty as charged, and meted the appropriate penalty.

1. I am complaining witness for serious Physical Injuries against Nick Carter in the case
entitled “People of the Philippines versus Nick Carter”, Criminal Case No.31254
Provincial Trial Court, Branch No. 7, City of Pagadian.
2. After my sober and soul searching assessment and analysis of the incident, I have
realized that because I was not wearing my eye glasses and it was dark, I cannot point
out, with a doubt the accused or any other person/who inflicted harm against me.
3. Since I could not state with certainly and without doubt the liability of Nick Carter,
in fairness to him, I am permanently withdrawing my complaint against him. I clear
him of whatever responsibility or liability to me.
4. I hereby inform the City Prosecutor of Pagadian that I am withdrawing my complaint
for Serious Physical Injury in Criminal Case No. 31254 entitled “People of the
Philippines versus Nick Carter”, Provincial Trial Court, Branch No. 7 City of
Pagadian.
5. I likewise request the Provincial Trial Court, Branch No. 7 City of Pagadian to
dismiss with prejudice the said criminal case.

We, the UNDERSIGNED Police Officers, both members of the Philippine Ntional
Police, presently assigned at Guipos Police Station, Pob. Guipos, Zamboanga Del Sur, do hereby
depose and state THAT:

01. We are among the members of the Guipos Police Station who conducted entrapment
operations at Drugs Shipment located along Barangay Bulawan San Miguel warehouse of the
rice milled, San Miguel Zamboanga del Sur.

02. Said operation stemmed from the complaint of PO3 Stephen Galadlas for violation of
Illegal drugs using which was referred to our office by the Recycle of Drugs Comfiscated
against to the Comprehensive Dangerous Drugs Act,(42),(married),(Filipino) and residing at
No.47241, Guipos Municipal Trial Court.

03. on the complaint dated 12th day of May 2022(using illegal drugs),

04. Upon receipt of their said complaint, our office planned out an entrapment operations
against the suspect. At about PO3 Stephen Galadlas of Guipos Police Station, the undersigned
together with the complainant proceeded at the prearranged pay off P200,000.00 Located along
Brgy; Bulawan San Miguel Zamboanga Del Sur to give the money being asked by the
respondent;

05. Upon the arrival thereat complaints waited for the suspect to arrive while we the
undersigned seated near the table of the complainants. At about 2 Hours, suspect arrived and
approached the complaints and after few conversation, complainants handed over the market
money to the suspect that minutes of prompted the undersigned to immediately effect arrest;

06. Seized and recovered in the possession and control of the arrested person were the
following to wit;

a.) Marked Money.(PTK 15763)

b.) Sworn Statement
c.) Tm Prepaid Card and;
d.) Picture and Fingerprint

07. Suspect together with the seized and recovered evidence was brought to the Guipos
Municipal Trial Court, Guipos Zamboanga Del Sur for booking and proper disposition, likewise,
the arrested person was apprised of their rights as provided for under the Constitution of the
Philippines.
 08. The report of the PNP Crime Laboratory Group Physical Identification Report No.
11547 dated 09-11-2020, revealed that suspect Susan Arnaud was POSITIVE for the presence of
Bright Yellow-Ultraviolet Fluorescent powder on both palm and night dorsal portion of the
hands.

We executed this joint affidavit to attest the truthfulness of the foregoing fact and to
support the filing of Criminal Cases against Nick Carter for violations of our law.

FACTS OF THE CASE:

Investigation disclosed that the victim and the suspect were long-time friends. On Nov, 18
2021, at around 7:30 p.m., two men together with several others had a drinking spree inside the
house of the victim. After consuming several bottles of beer, a heated altercation ensued between
one of the victim’s guest and the suspect. The victim was conveyed to the JRRMH in Pagadian,
but was pronounced DOA by the attending SOD. The suspect, who was apprehended by the
responding officers, was turned over to this officer for investigation.

On account of the foregoing, the suspect was placed under arrest and the charge of
HOMICIDE against him will be brought to the attention of an inquest fiscal for proper
disposition and recommendation.

motorcycle model Raider 125 color Red.
IV. Pagkatapos kong makita ang insidente ay tumawag ako sa Guipos Police
Station para sa insidenteng nangyari sa Purok 5 Guling, Zamboanga Del Sur.

Isinagawa ko ang affidavit na ito upang patunayan ang katotohanan ng mga naunang
katotohanan at upang suportahan ang pagsasampa ng Mga Kasong Kriminal laban kay Ariel
Bustard para sa paglabag sa Murder.

01. Kami ay kabilang sa mga miyembro ng Guipos Police Station na nagsagawa ng

entrapment operations sa Drugs Shipment na matatagpuan sa kahabaan ng Barangay
Bulawan San Miguel warehouse ng rice milled, San Miguel Zamboanga Del Sur.
02. Ang nasabing operasyon ay nag-ugat sa reklamo ni PO3 Stephen Galadlas dahil sa
paglabag sa paggamit ng Iligal na droga na ini-refer sa aming tanggapan ng Recycle of
Drugs Comfiscated against to the Comprehensive Dangerous Drugs Act,(42),(married),
(Filipino) and naninirahan sa No.47241, Guipos Municipal Trial Court.
03. Sa reklamo na may petsang ika-12 araw ng Mayo 2022(paggamit ng ilegal na droga),
04. Nang matanggap ang kanilang nasabing reklamo, nagplano ang aming tanggapan ng
entrapment operations laban sa suspek. Sa humigit-kumulang PO3 Stephen Galadlas ng
Guipos Police Station, ang mga nakapirma kasama ang nagrereklamo ay nagpatuloy sa
nakatakdang pagbabayad ng P200,000.00 Matatagpuan sa kahabaan ng Brgy; Bulawan
San Miguel Zamboanga Del Sur na ibigay ang perang hinihingi ng respondent;
05. Pagdating doon ay naghintay ang mga reklamo na dumating ang suspek habang kaming
mga nakapirma sa ibaba ay nakaupo malapit sa mesa ng mga nagrereklamo. Humigit-
kumulang 2 Oras, dumating ang suspek at nilapitan ang mga reklamo at pagkatapos ng
ilang pag-uusap, ibinigay ng mga nagrereklamo ang pera sa palengke sa suspek na ilang
minuto ng pag-uudyok sa nakapirma sa ilalim na agad na arestuhin;

may petsang 09-11-2020, ay nagsiwalat na ang suspek na si Susan Arnaud ay POSITIBO
sa pagkakaroon ng Bright Yellow-Ultraviolet Fluorescent powder sa magkabilang palad
at gabing bahagi ng dorsal ng mga kamay.

Isinagawa namin ang pinagsamang affidavit na ito upang patunayan ang katotohanan ng
naunang katotohanan at upang suportahan ang pagsasampa ng Mga Kasong Kriminal laban kay
Nick Carter para sa mga paglabag sa ating batas. .
