PHILIPPINES, INC, doing business under the name and style ZALORA, is filing an
Application for Authority to Operate as an International/Domestic Airfreight Forwarder.

This application contains the following attachments:

1. Verified Application;
2. Certificate of Registration from Securities and Exchange Commission (SEC) &
Articles of Incorporation;
3. Latest General Information Sheet;
4. Insurance Policy covering Office/Property and the corresponding proof of
premium payment made issued by the insurance company; and
5. Certified True Copy of the Audited Financial Statements.

General Information Sheet of ZALORA was recently updated and an authenticated copy of the
same is not yet available from the SEC. A copy of the acceptance email dated from the SEC
dated 10 January 2023 is likewise attached in this application.

At the moment, ZALORA is still in the process of searching for CAB Accredited Training
provider. The Certificate of Training in Dangerous Cargo or Certificate of Enrolment and Copy
of Official Receipt of Enrolment Fee of Employee will be submitted at a later date as soon as the
Certificate of Enrolment is available.
