from the day you officially
resigned/terminated
 PREPARATION
Download the checklist to reapply
Download and fill up the attachment H
Request for C2/ C5 and C6 attachments from
your previous hospital
Duration 1-3 days
 MMC REVIEW
After cooling period is over, you may submit
attachment H to MMC review
Duration 1-2 weeks
Call MMC if no update after 2 weeks
 BOOK APPOINTMENT
WITH KKM
After collecting your attachment H from MMC
Call Dr Siti Rahayu's PA (Cik Aisyah) to set an
appointment. (Bahagian Perkembangan
Perubatan)
You will be put in a waiting list
Duration 4-5 months
 IV WITH KKM
Interview date informed via call or email
Bring all the attachments + a copy of
provisional MMC certificate + a copy of previous
appointment letter
Dr Siti Rahayu will sign on the attachment H
 SUBMIT TO HR KKM
Submit all the documents straight to KKM
(Bahagian Sumber Manusia) Blok E7, Level 9
You will be ask to call them for update, whether
they have submit our application to SPA or not
Duration 1-2 weeks
 IV MRP
For those under TCA PSY or any medical issues
Waiting period >5months
MMC will ask for medical report (valid for 3
months only)
In most cases, need to request new medical
report from hospital
Usually takes 1 month to prepare medical
report (sometimes less), depends on your issue
 SPA
Upon confirming KKM has submitted your
application to SPA
You may fill up your application at SPA8i
Then you just sit tight and wait for eHO
Duration 6-9 months (depends on intake)
