AFFIDAVIT OF DENIAL

I, EDGAR OBIADO BALUYOT, 53 years old, married, Filipino and a resident

of Brgy. Rawis, Laoang, Northern Samar, after having been sworn to in accordance
with the law, hereby depose and state:

1. That I applied for an NBI Clearance at the NBI Tacloban Satellite

Office on _____; however, I was not issued a clearance by the said
office because based on their database, there is an existing name
similar that of mine having a pending case before the Regional Trial
Court, Branch 21, Laoang, Northern Samar.

2. That I vehemently deny the accusations against me with regard to

the Criminal Case No. ___ for Frustrated Murder, which is pending
before the Regional Trial Court, Branch 21, Laoang, N. Samar.

me because my real and complete name is EDGAR OBIADO
BALUYOT.

for whatever legal intents and purposes this may serve.
