JOINT AFFIDAVIT OF ARRESTING OFFICER

We, the undersigned Police Officer and presently detailed as Patroller Police Non-
Commission Officer (PNCO) of Cauayan City Police Station, Barangay San Fermin, Cauayan
City, Isabela after having been sworn to an oath in accordance with law, do hereby deposed
and state the following to wit;

That at about 9:00 AM of November 7, 2021 we conducted Anti-Criminality Checkpoint

along Maharlika Highway, Brgy. Tagaran, Cauayan City, Isabela. Led by PMAJ SHERIE-ANN
G MASI DCOP Cauayan City Police Station, while conducting the said checkpoint operation
one (1) Alfred Vergara y De Leon was approaching to the checkpoint not wearing his helmet
and driving recklessly.

Acting on the said report, Patrolman Doctor and Patrolman Almondia flagged down the
said motorcycle rider to verify the identity of the driver upon the verification we asked him to
present his OR/CR, and while getting his OR/CR on his compartment we saw the allegedly
white crystalline packed in a small sachet, there and then Patrolman Almondia confront the
driver about the white crystalline packed in a small sachet and immediately arrested for proper
disposition.

That we learned that all involved person/s and vehicle/s were: Yamaha Sniper 155
bearing with temporary plate # 0001-0000123 registered to ALFRED VERGARA y DE LEON.,
male, 25 years old, single, construction worker and resident of Brgy. Dadda, Amulung,
Cagayan.

That we brought the arrested suspect ALFRED VARGARA y DE LEON together with his
driven motor vehicle YAMAHA SNIPER 155 at Cauayan City Police Station for proper
disposition and documentation and after which I effected his arrest and informed of his
constitutional rights in compliance with the Miranda Doctrine in a dialect understood by him.

That, I executing this affidavit to attest the truthfulness of the foregoing facts and
declarations;
