AFFIDAVIT

I, ATTY. LYN MULAAN-OLITA, of legal age, Filipino, married, and a resident of Brgy.
Tunga-tunga, Maasin City, Southern Leyte, Philippines, after having been duly sworn to in
accordance with law, depose and say:

1. That I personally know NOEL ORAG DOLERA, Filipino, of legal age, and a resident of
Mantahan, Maasin City, Southern Leyte;

2. That he was born on December 24, 1990 to Carmencita Orag Dolera and Ireneo
Markines Dolera;

3. That he applied for clearance from the National Bureau of Investigation Maasin City
Satellite Office;

4. That the said office could not issue the required clearance in view of the appearance of
a name similar to his, and based on NBI files, the person bearing the same name is
criminally charged with unjust vexation before the Municipal Court of Tubigon, Bohol,
Branch 02A, and that said person’s name is NOEL DOLERA, without a middle name;

5. That his true and full name is NOEL ORAG DOLERA, which is different from the one
appearing on their records;

6. That NOEL ORAG DOLERA has been living in Mantahan, Maasin City, Southern Leyte for
31 years, and in fact, has never set foot in Tubigon, Bohol;

7. That I have known NOEL ORAG DOLERA, since we were schoolmates for a very long
time;

8. That I am executing this affidavit to attest to the veracity of the foregoing, and to
confirm NOEL ORAG DOLERA’S identity and personal information, and for whatever
legal purpose this may serve, particularly for the issuance of an NBI clearance.
