AFFIDAVIT OF HEIRSHIP

I, MARLON G. MENDEZ, of legal age, married, Filipino and a

resident of Brgy. Occidental II, Gamay, N. Samar, after being sworn to in
accordance with law, hereby depose and say:

1. That I am one of the children of Sps. Eutiquio G. Mendez

and Maria G. Mendez of Gamay, N. Samar.

2. That my father, Eutiquio G. Mendez, has already passed

away on August 22, 2013.

3. That my father died without a Will, either holographic or

notarial.

4. Thus, as one of his children, I am one of the compulsory

heirs of my father, Eutiquio G. Mendez.

5. That I executed this affidavit to attest the truthfulness and

veracity of the foregoing facts and for purposes of claiming
the PCA Cash and Food Subsidy for coconut farmers, and
for whatever legal purpose this may serve.

IN WITNESS WHEREOF, I hereunto set my hand this 9 th day of

February 2021, in Gamay, Northern Samar.
