This Document has been issued to highlight the general Policy & Procedures of Sky
Vision Airlines Quality System.

In accordance to our vision to become a leading world-class airline, we at Sky Vision


Airlines are committed to safety and quality as our most important core elements.

Safety and quality are integral and essential elements of every process at Sky Vision
Airlines. We consider safety and quality to be vital characteristics of our products,
intimately linked to how we perform our activities.

This manual is the tool of the Accountable Executive of Sky Vision Airlines for setting
policies and standards for Quality system oversight, to ensure that elements of quality
management system are applied in all Sky Vision Airlines activities.

Our customers are foremost in our minds all the time. We go the extra mile to exceed
their expectation.

The contents of this manual are approved by Accountable Executive and must be
complied by all parties within Sky Vision Airlines

Directors and Managers are accountable for practical implementation and continuous
enforcement.

This manual does not require the approval and acceptance of the Authority; it will be
internally approved by the Quality Assurance Manager and the Accountable Executive.

0.3. Introduction:
This manual is designed to ensure control of Quality Assurance of our operations and its
documentation in accordance with requirements, regulations and standards of various
authorities including the Egyptian Civil Aviation authorities (ECAA), relevant foreign
aviation authorities, IOSA standards, and applicable international regulatory bodies. In
addition, the manual aims to ensure full compliance with our own internal requirements for
high standard. This manual Contains the Internal Evaluation Program (IEP) Audit procedure, and
audit check list of all the company Departments & External Service Provider. The Procedure for
corrective actions, in case of any discrepancy in the system before it may occur

0.13. Definitions
 Acceptable level of safety performance (ALoSP). The minimum level of safety performance
of civil aviation in a State, as defined in its state safety program, or of a service provider, as
defined in its safety management system, expressed in terms of safety performance targets
and safety performance indicators.
 Accident. An occurrence associated with the operation of an aircraft which takes place
between the times any person boards the aircraft with the intention of flight until such time
as all such persons have disembarked, in which:
- a person is fatally or seriously injured as a result of: being in the aircraft, or having direct
contact with any part of the aircraft, including part which have become detached from the
aircraft, or direct exposure to jet blast.
- The aircraft sustains damage of structure, which adversely affects the structural strength,
performance or flight characteristics of the aircraft, and would normally require major repair
or replacement of the affected component.
- The aircraft is missing or is completely inaccessible.
 Accident Prevention. The detection and the elimination or avoidance of hazards to prevent
accidents, thus improving public confidence in Air safety, saving lives, property and money
and reducing suffering.
 Accident scenario: continuation and/or combination of circumstances encouraging the
occurrence of events that ultimately cause an accident. An accident scenario is a
combination, not necessarily a logical continuation, whose initial origin is the presence of a
hazard that, following a specific context, can result in an accidental situation or an accident.
An accident scenario is also characterized by the severity of the consequences of the
accident that it causes.
 Accountable executive” CEO”. The single, identifiable senior management official designated
by an organization as having responsibility for the operational performance of an
organization's management system.
 Acts of Unlawful Interference. Any criminal act which jeopardizes the safety, regularity, or
efficiency of international civil aviation.
 Agreement. A written statement accepting certain conditions.
 Aircraft – Ground Incident. An occurrence associated with the operation of an aircraft
involved in an emergency situation in which the aircraft or a person has been exposed to
undue risk resulting in an emergency evacuation of passenger / crew by emergency slides,
injuries to persons involved, and / or damage to the aircraft. Aircraft accidents in which there
are no serious injuries or deaths may also be classified as an aircraft – ground incident if
there is major damage to the aircraft.
 Air Operator Certificate (AOC). A certificate issued by the state of the operator authorizing
the operator to carry out specified commercial air transport operations involving the

transport of passengers, cargo, mail and / or baggage for remuneration or hire (ref. ICAO
Annex 6, Part I).
 Air Traffic Control (ATC). The local airport facility of Air traffic services which controls aircraft
movement within a designated geographical area of the state.
 Airworthiness Certificate. A mandatory certificate issued and validated by the state of
registry to verify compliance with minimum ICAO standards to authorize eligibility of the
aircraft for international operations (ref. ICAO Annex 8).
 Audit. Systematic and independent examination to determine whether safety and quality
activities and related results meet requirements based on documentation, procedures and
work instructions and whether procedures are implemented effectively and are suitable to
achieve desired objectives.
 Certificate. A written statement of qualifications or accomplishments.
 Change management. A formal process to manage changes within an organization in a
systematic manner, so that changes which may impact identified hazards and risk mitigation
strategies are accounted for, before the implementation of such changes.
 Civil Aviation Authority (CAA). A government – appointed agency which regulates oversees,
and ensures the safety of air transport within the territory of the state.
 Competency. Processing adequate and suitable qualifications and skills to perform a
particular task, duty, or profession.
 Continuation Training. Training which supports and builds on a foundation of previous
training and existing skills.
 Convention on Internal Civil Aviation. The constitution of ICAO established at the Chicago
convention of 1944 by signatory states to establish regulations and obligations of states to
ensure safe international air transport (ref. ICAO Doc 7300).
 Corrective action. Remedial action intends to correct/improve/overcome any defect or
nonconformity in documentation, procedures or implementation.
 Customer. One who buys or purchases products, goods or services from another.
 Defenses. Specific mitigating actions, preventive controls or recovery measures put in place
to prevent the realization of a hazard or its escalation into an undesirable consequence.
 Department head. The administrative head with responsibility and authority over a major
area of Sky Vision Airlines business activities.
 Emergency. A state of sudden, pressing necessity requiring immediate response.
 Emergency evacuation. The urgent and rapid disembarkation of occupants from the aircraft
due to imminent danger to the aircraft and its occupants.
 Errors. An action or inaction by an operational person that leads to deviations from
organizational or the operational person’s intentions or expectations.
 Federal aviation administration (FAA). The CAA of the United States empowered to

 Hazard. Potential for damage or harm to individuals, goods or the environment. A hazard is a
situation that could seriously affect the physical integrity of a person. The terms hazard and
risk are commonly confused.
The nature of the hazard may be tangible (mechanical device, chemical substance, virus,
etc.) or intangible (potential or kinetic energy, incomplete training, etc.).
The hazard may be the property of a substance (toxic chemical, etc.), a material object
(machine, virus, etc.) or a situation (social conflict, ill-defined or poorly implemented
process, such as human error, misdiagnosis, wrong strategy, management mistake, etc.).
Knowledge of the nature of a hazard is the starting point for analyzing the risks.
In this manual the terms hazard, threat and Unsafe State are used interchangeably.
 High-consequence indicators. Safety performance indicators pertaining to the monitoring
and measurement of high-consequence occurrences, such as accidents or serious incidents.
High-consequence indicators are sometimes referred to as reactive indicators.
 Human Factors. Factors dependent on individual human judgment or response which could
contribute to the possibility of the occurrence of an incident or accident
 Incident: an unforeseen event during the operation of a system or the performance of an
activity, the consequences of which are a system malfunction, disruption to the activity or
the occurrence of mild material damage. Regardless of the circumstances, the consequences
of an incident are no more serious than minor malfunctions or equipment damage.
 Inspection. Activity such as measuring, examining, testing or gauging one or more
characteristics of products or services and comparing the results with specified
requirements in order to establish whether conformity is achieved for each characteristic.
 International Air Transport Association (IATA). A non-governmental agency participating in
the work of ICAO by supporting ICAO standards and recommended practices and by
promoting safe practices in the aviation industry.
 International Civil Aviation Organization (ICAO). A body of signatory or contracting states
which establishes the obligations and responsibilities of states to ensure safe international
air transport for all carriers operating within their respective territories.
 Joint Aviation Authorities (JAA). The joint CAA of European states empowered to monitor,
control, and enforce aviation safety to / from / through the territory of Western Europe.
 Joint Aviation Requirements (JAR). Special requirements of Western European States which
air carriers must comply with in addition to minimum requirements of ICAO for international
commercial air transport to / from / through the territory of Western Europe.
 Key Performance Indicator (KPI). Aspects of performance with major impact on customer
satisfaction and business activities which are measured on a regular basis to track
performance for continual improvements in safety and quality.
 License. A legal permit authorizing one to perform a particular activity or engage in a
particular profession.
 Lower-consequence indicators. Safety performance indicators pertaining to the monitoring
and measurement of lower-consequence occurrences, events or activities such as incidents,
non-conformance findings or deviations. Lower-consequence indicators are sometimes
referred to as proactive/predictive indicators.

 Maintenance. The routine servicing, repair and inspection of aircraft and engines to ensure
safe operation.
 Monitoring. Keeping regular watch over activities to track performance.
 Non-conformance. Any deviation from or failure to comply with established policy,
procedures, documentation, or work instructions.
 Organizational Structure. The structure of the Sky Vision Airlines organization defining roles,
responsibilities, authority, channel of communication and the chain of command.
 Preventive Action. Any action or major intended to prevent hazards from developing which
could otherwise result in accidents
 Process. A series of steps, actions, or majors which take and input and modify it to produce
and output.
 Product. The final output, outcome, or commodity resulting from a particular process.
 Quality. Compliance with requirements; total customer satisfaction.
 Quality Assurance. A function which monitors and ensures the quality of work performed
within a particular department in compliance with requirements.
 Recurrent / Refresher Training Risk. The consequences of accepting a hazard
 Risk analysis. An analysis for assessing an existing safety level (specific operating conditions,
processes, procedures, equipment, etc.) in order to ensure that risks remain under control or
in response to a change or development in order to assess the risks associated with its
implementation.
 Risk mitigation. The process of incorporating defenses or preventive controls to lower the
severity and/or likelihood of a hazard’s projected consequence.
 Safety: Situation in which the risks of bodily injury or material damage are reduced to an
acceptable level and kept at or under this level by a continuous process of identifying
hazards and managing risks.
 Safety action: an action taken by a department or entity of the organization with a view to
improving safety and which results from an investigation, an audit or a safety study. This
term includes the words recommendation, corrective action and preventive action.
 Safety management system. A systematic approach to managing safety, including the
necessary organizational structures, accountabilities, policies and procedures.
 Safety barrier: a hardware device, software device, human factor and/or organizational
device designed to counter the occurrence of an undesirable event or slow the evolution of
an accident scenario. The barrier concept is very similar to that of the safety principle.
 Safety Oversight. A function by which states ensure effective implementation of safety
related ICAO standards and recommended practices (SARP) and associated procedures or
documents contained in the Annexes (and related documents) of the convention on Internal
Civil Aviation; safety oversight ensures that the aviation industry provides a level of safety
equal to or better than defined by SARP.
 Safety performance. A State’s or service provider´s safety achievement as defined by its
safety performance targets and safety performance indicators.

 Safety performance indicator. A data-based safety parameter used for monitoring and
assessing safety performance.
 Safety principle: A safety measure forming part of our operations on which the
Man/Machine system, in its broad sense, relies and which plays a part either in the
prevention of or recovery from an Undesirable Event, or in mitigating the consequences of
an accident. It is defined as a hypothesis/assumption that some factor (technology,
procedures, training, behavior, organization, etc.) contributes to the safety of a system.
 Safety risk. The predicted probability and severity of the consequences or outcomes of a
hazard.
 State safety program. An integrated set of regulations and activities aimed at improving
safety.
 Security Threat. A potential hazard to the security of operations which could harm or
endanger property, human life, or the safety of operations.
 Service. Work performed to benefit another individual, company or organization.
 Standards. An accepted level of performance.
 State. A country as defined by legal boundaries and territory under the control of a formal
government.
 Supplier. A person, company or organization who/which has agreed to provide products or
services for the benefit of another.
 Threat. Potential for damage or harm to individuals, goods or the environment.
A threat can be regarded as the trigger of an accident scenario in the presence of a hazard. It
also affects the reliability of our safety principles. A threat can be regarded as an unsafe
state.
