LETTER FOR REQUEST TO CONDUCT SURVEY AND OBTAIN INFROMATION

In partial fulfilment of the requirement of Bachelor Science Human Resource Development

student (S.Y. 2014-2018) in Thesis in Human Resource Development. I would like to request
your cooperation to allow me to conduct a survey among the management and members in Air
Asia airline organization. This is in view of my thesis, entitled, “Relationship between
Organizational Citizenship Behaviour (OCB), Work Life Balance (WLB) and employee
performance”.
Regarding this study, some of informations regarding airline organization and workforce
structure are also require for this study. I will distribute the survey questionnaires to management
people and members of airline organization that will help me to obtain information which are
relevant to my study. The survey questionnaire encompasses by questions related to
organizational citizenship behaviour and work life balance of employees.

The survey would last only about 10-15 minutes and would be arranged at a time convenient to
the employee’s schedule (e.g. during break). Participation in the survey is entirely voluntary and
there are no known or anticipated risks to participation in this study. All information provided
will be kept in utmost confidentiality and would be used only for academic purposes. The names
of the respondents and the name of your organization will not appear in any thesis or
publications resulting from this study unless agreed to.

After the data have been analysed, you will receive a copy of the executive summary. If you
would be interested in greater detail, an electronic copy (e.g. PDF) of the entire thesis can be
made available to you.

I would greatly appreciate your consent at my request. Thank you in advance for your interest
