    We, the parents of _____________________________ from class of Grade 10 – St. Anthony de Padua
    would like to request your good office to kindly allow our child to bring his mobile phone to school for
    reason stated below:

    Reason (s):
    Please consider that our house which is located in _(address)__ _______ is too far from the
    school, so we feel more secure if we can track him at the time of arriving and leaving the school premises
    as he only commutes to school and back home.

    Mobile Phone Model: iphone 11 Tracking App: icloud (find my phone)

    We are aware that if the rules on the use of mobile phones during restricted time or while inside the
    school premises are violated, the mobile phone can be confiscated by the SBO and this privilege will be
    revoked.

    We also acknowledge that the school will not be held responsible for the loss of the mobile phone.
