This is in reference to your query about allowing my ward to appear for offline annual
examinations in school, scheduled to start from 08th of March, 2021.

son/daughter permission to go to school to appear for the annual examination. I will
ensure that my son/daughter strictly follows the norms of social distancing, mask
wearing, hand washing/sanitizing and other precautions set in the Standard Operating
Procedure of the Government of Uttarakhand and Ann Mary School, to safe guard against
COVID 19. I am providing the mobile no.s (02) which shall be reachable in case of any
emergency during the school hours.

INSTRUCTIONS FOR PARENTS REGARDING THE CONSENT LETTER

1. The printout of the letter to be taken out filled, scanned and mailed to the below
mentioned e-mail address:
annmaryschoolddn@yahoo.in

2. A hard copy of the form, duly filled can also be submitted in the school front
office (gate No.3) between 10:00 to 1:00pm during working days.

3. The last date for submission of the consent letter is Friday the 19th of February,
2021
FOR PARENTS
CLASS - XI
Dear Parent,
Kindly ensure that the instructions given below are strictly followed by your child.

1. No entry without mask.


2. Entry will be through gate no. 1 (GMS Road) and Gate no. 8 (Ekta Lane). The arrows
and circles for social distancing need to be strictly followed.
3. Only students who have submitted the consent letter will be allowed to appear for
the preliminary examination.
4. Students should turn out neatly in their winter uniform (haircut, beard to be taken
care of).
5. Students should carry their own water bottle, stationery items and sanitizer and
handkerchief.
6. Borrowing or lending of items is strictly prohibited.
7. Arrival of students should be not earlier than 9:30am and later than 9:50am.
8. The seating plan (place where the student is seated room, desk no.) is being shared
on the what app. group by the class teachers. This has to be followed strictly.
9. Students should not carry any text book/exercise book/ bag or writing pad, this will
help maintain contact less conduct of exams.
10. Parents coming to drop and pick up children must also observe precaution against
Covid – 19 like mask wearing and social distancing.
