Good day dear CMULHS students!

Our school holds PRE-ENROLMENT activities at the end of each School Year
(SY) to facilitate the enrolment process for the upcoming academic year. Students are to
fill-up the CMULHS Enrolment Form* (link will be provided by respective Section
Adviser) to update their personal data. This is also the time for students and their
parents/guardians to view the Student Report Card (Form 138) for the current school
year. Completing the process signifies the student’s intent in enrolling to CMULHS for the
upcoming SY 2023-2024.
Specific instructions are as follows:
1. Download the fillable form [Grade]-Enrolment.pdf by clicking the link below. It
is a two-page form. Make sure that you download the file for your Grade Level
NEXT school year, and NOT YOUR CURRENT grade level.

2. Fill out the fields in the form with appropriate information.
3. Review your answers. Note that there are some fields which are required.
4. Save the file with a different filename so that you can have your own e-copy.
Example: CRUZ-G12-Enrolment.pdf
5. Print your completed Enrolment form in a single Long (8.5 x 13 inches) bond
paper, printed front and back. Before you click the print button, make sure that
you selected the appropriate printer and paper size. This is important as your
printed copy may not be accepted if it will not follow the prescribed format and
paper size. If you need assistance in printing instructions, please contact your
Section Adviser.
6. Bring your printed Enrolment form to school on your scheduled date.
• June 14, 2023 – Grades 10 &12
• June 15, 2023 – Grades 7 & 11
• June 16, 2023 – Grade 8 & 9
Note: The fillable Form can also be filled out first before you download it for printing.
