Parent’s Consent Letter

I, Mejeda D. Caacbay, give my consent for my child Salvador D. Caacbay Jr. to participate on
conducting an interview in our barangay as part of their academic requirement for the course
PAS6: Local and Regional Governance. The Interview will take place on May 13, 2023
I understand that my child’s participation in this interview is voluntary and may involve some
degree of risk. However, I am confident in my child’s abilities and trust that they will conduct
themselves in a responsible and respectful manner during the interview.
I hereby release Rosenne S. Salvador, MPA, and the Palawan State University – College of
Business and Accountancy from any and all claims, damages, and liabilities that may arise from
my child’s participation on the interview.
