Ive created a RCA Dashboard for us to easily analyze the errors and root causes of the unacceptable files.
It is done on a yearly basis.For 2021, It has a total of 6562 errors; the frequent error type is word usage
vocabulary; the kind of error is usually a language error; the location of the error is in the results and
discussion, and introduction. The month with the most frequent number of feedback received is
October. The root cause of the errors is misjudgment—retaining words that need to be changed.
