Vigabatrin (Sabrilex®):
Farmakodynamik
Vigabatrin är ett antiepileptikum med väldefinierad verkningsmekanism. Behandling
med vigabatrin leder till en ökning av koncentrationen av GABA
(gammaaminosmörsyra), den viktigaste inhibitoriska neurotransmittorn i hjärnan.
Vigabatrin är en selektiv, irreversibel hämmare av GABA-transaminas, det
enzym som ansvarar för nedbrytningen av GABA. […]
Farmakokinetik
Vigabatrin är en vattenlöslig substans som snabbt och fullständigt absorberas från
magtarmkanalen. Födointag påverkar inte graden av absorption. Läkemedlet
distribueras i stor grad med en distributionsvolym något större än den totala
volymen av vatten i kroppen. Cerebrospinalvätske- och plasmakoncentrationer
är linjärt proportionella mot dosen inom rekommenderat dosområde.
Det finns ingen direkt korrelation mellan plasmakoncentration och effekt. Durationen
av läkemedlets effekt beror på hastigheten för resyntes av GABA-transaminas.
Vigabatrin elimineras från plasma med en halveringstid på 5-8 timmar. Cirka 70% av
en peroral engångsdos utsöndras oförändrat i urinen under de 24 första
timmarna. Inga metaboliter har identifierats. […]

Esomeprazole ®
(Nexium ):
Farmakodynamik
Esomeprazol, S-isomeren av omeprazol, hämmar syrasekretionen i ventrikeln
genom en målstyrd verkningsmekanism och hämmar specifikt syrapumpen i
parietalcellen. Både R- och S-isomeren har likartad farmakodynamisk aktivitet.
Verkningsmekanism:
Esomeprazol är en svag bas som koncentreras och omvandlas till aktiv form i den
mycket sura miljön i parietalcellens sekretoriska kanaler, där den hämmar
enzymet H+K+-ATPas, d v s det sista steget i syrasekretionsprocessen och ger
en hämning av såväl basal som stimulerad syrasekretion.
Effekt på syrasekretionen: […] Efter peroral dosering med 20 mg och 40 mg
esomeprazol i 5 dagar hos patienter med symtomatisk GERD bibehålles
intragastriskt pH över 4 under i genomsnitt 13 respektive 17 timmar av en 24
timmars period. Andelen patienter, hos vilka ett intragastriskt pH över 4 bibehålls
i minst 8, 12 respektive 16 timmar är för esomeprazol 20 mg 76%, 54%
respektive 24%. Motsvarande för esomeprazol 40 mg är 97%, 92% respektive
56%.
Ett samband mellan syrasekretionshämning och exponering kan ses om AUC
används som surrogatparameter.

Farmakodynamik
Fentanyl är ett opioidanalgetikum med affinitet framförallt till -
receptorn. Dess huvudsakliga terapeutiska effekter är
smärtlindring och sedering. Lägsta serumkoncentrationerna av
fentanyl som ger analgetisk effekt hos opioidnaiva patienter
varierar mellan 0,3 och 1,5 ng/ml, och en ökad
biverkningsfrekvens observeras när serumkoncentrationerna
överstiger 2 ng/ml. Både den lägsta effektiva
fentanylkoncentrationen och koncentrationen vid vilken
biverkningar inträffar höjs med ökande tolerans. Benägenheten
för toleransutveckling varierar kraftigt mellan olika individer.
