TO WHOM IT MAY CONCERN:
We, the teachers, and non-teaching staff of TINAGAAN INTEGRATED SCHOOL whose
name, signature and designated heir found below hereby certify/affirm our commitment to
join as member of Project 25. That in the event of LOSS of a teacher non-teaching staff, we
willfully give and commit ourselves to share 25 pesos.
And in the event of our retirement, we present the GSIS clearance as effectivity of our
retirement and a Lifetime Member of Project 25 (as approved by laws, last November 21, 2022).
