AFFIDAVIT

I, OSIAS Z. SEMBLANTE, of legal age, Filipino, married and a resident of P-5,

Brgy. Calibunan, Cabadbaran City, Agusan del Norte, Philippines, after having
been duly sworn to in accordance with law, hereby depose and say:

1. That I am the driver of Mr. POLICARPIO QUIMNO for almost 6 years;

2. That on March 13, 2022, I loaded forest products using the vehicle with
plate no. 124696 registered under the name of Mr. Quimno;

3. That on April 18, 2022, I attended a Summary Administrative

Proceedings at CENRO Nasipit and personally told the hearing officer
and the members of technical working group that I did the act of
transporting the forest products without documents to earn money for
personal gain.
