Dear Sir,

Complaint against the Health Worker, Smt. Sammeti Sailaja (Mobile No. 9618386001)

I am Arni R. Rama Rao aged 78 years, a GoI pensioner living in Satyanarayanapuram

with my son, Arni Purushottam.

My wife, Smt. Padmavati Arni had a fall in the bath room and suffered a fracture in the
hip bone during the days of Corona endemic and was confined to bed 21.05.2020.
After being in this condition, she finally passed away on 18 February 202 after a
continued treatment and medical attention. Dr.Srinivas of Gannavaram was treating her
during that period. To provide nursing and to attend on her at our home in
Satyanarayanapuram, Smt Sammeti Sailaja, a health worker working in the hospital of
Dr.Srinivas was engaged. The said doctor was sending medicines through Sailaja. The
health worker worked for about 2 years. She was paid a wage of Rs.1,0000/- per month
during this period.
The said Smt Sailaja during the periods she was nursing my wife and thereafter too,
took on and off monies from me on various pleas amounting to a total of Rs. 1,04,000/- (
as per list enclosed). These amounts were paid through Phone Pe to her mobile
number 9618386001 from my son’s mobile number 6305456200.
During the past few months, she over phone told me that she got the amount of
insurance amount due to me on account of death of my wife, collected by her and lying
in her above bank account number 2425101019153 with Canara Bank. She keeps
telling that she will come to Vijayawada and pass on the insurance money to me, and
held up for one reason or the other.

She later wanted details of my bank account with SBI to transfer money into the
account. Though, I gave her the details of my bank account (image of first page of the
pass book), she never remitted the money to my account.

Till now, she has neither passed on the amount taken from me to my bank account as
promised nor returned in cash.

Of late, She does not pick up calls made by me. Recently when inquired on whatsApp
message, she says she is in Rajahmundry and busy with some land deal.

I request you to kindly look into the issue and help me in getting back the amounts paid
by me as also the insurance amount stated to have been received by Smt Sammeti
Sailaja due to me on account of wife’s death.
