DEDICATION
We would to like dedicate the Doctors, Nurses, Patient, Admin and also to our
Professor for supporting our project and this project belongs to the people of Pateros
that would appreciate our project by making a Schedule in E-consult or Face to Face
Appointment in to the Quintin De Borja Health Center. Above all, to our Almighty God,
who has showered us with His benefits in our daily lives, particularly for the strength,
courage, patience, wisdom, time, and guidance in carrying out our mission.

We would like to acknowledge Quintin De Borja Health Center for supporting our
project by gathering some information given by the Doctors and Nurses in their Health
Care Center and we would to acknowledge our professor, Prof. Rommel Dorin for
supporting our project by valuable guidance, Instructing, and words of encouragement
to us. We want to Thank You for all of your support, guidance, instructing, and
encouragement you gave to us.

Module 1:
A. Project Description
The patient can save time for a face-to-face appointment and send their health
status to their doctor at an e-consultation appointment based on their health
status.
Patients are asked questions about their symptoms, and the survey only takes a
few minutes. The completed form is called a health questionnaire.
After the patient adds some personal information to verify their identity, the
eConsult system first uses built-in clinical knowledge to determine if help is
needed faster than the practice, e.g., B., can afford. Emergency services.
In these cases, the system stop processing the request and suggests how
urgently medical advice can be sought.

B. Roles and Responsibilities
When you phone the doctor's office to make an appointment, it can often take
hours, days, or even weeks to be seen. Of course, this is not acceptable for
many people, particularly because waiting extended times to see a doctor for a
medical concern can be stressful. Instead, you can access Quintin De Borja
Health Center Information System from your device. This is also a very useful
service for the elderly or those who are frail and cannot easily travel to the
doctors from their homes.
