1. The undersigned confirm that HE/SHE is duly authorized to sign this form on behalf of the client.
2. You AGREE to follow APM TERMS & CONDITIONS as state on the quotation and shall be
applicable to this request.
 APMCC turnaround time for the calibration is 10 - 15 Working Days (Mon – Fri, Excluding the
Public Holidays) upon received of the item with complete necessary documentation or details
required. Turnaround for customer who sending more than 5 units will be subjected for
discussion.
 APM reserves the right to return any device if they are found to be unsuitable / out of tolerance for
calibration.
 APM will do the adjustment for the survey meter based on manual operation instruction if necessary,
depend on customer permission.
 APM ONLY provide calibration service and not repairing service
 APM will not bear responsibility for any device broken during delivery to / from APM Calibration
Center
 APM will keep CONFIDENTIAL information in trust and strictly confidence and agree that the
information shall be used only for calibration purposes, and shall not be used for any other purpose, or
disclosed to any third party unless required by Law or authorized by customer.
 APM will charge RM40 for RE-Print Calibration Certificate if there is amendment caused by customer
mistake.
