Baseline Chart for thoughts


Thoughts baseline chart was used to help the client manage her intrusive thoughts
about performing same activity that is washing her hands repeatedly, and over thinking
about her divorce successfully. The client feels that she is being neglected and stopped
from doing something important to her and disturbing her everyday life. Thoughts
baseline chart was given to the client for week duration. It was given to assess the
client’s thoughts feelings and their frequency duration and intensity. It showed a pattern
of negative thoughts of the client about others. It also showed that how he gets into
worried thoughts and what do she do in that situation and what makes her calm down.

Baseline Chart for Compulsions

Compulsive behaviour baseline chart was used to help the client manage her compulsive
behaviour about performing same activity that is washing her hands repeatedly. The
client feels that she is being neglected and stopped from doing something important to
her and disturbing her everyday life. Thoughts baseline chart was given to the client for
week duration. It was given to assess the client’s compulsions, feelings and their
frequency duration and intensity. It showed a pattern of washing her hands over and over
again and never get satisfaction and urge to clean her surroundings.
