INFORMATION FOR CANDIDATES:

The following are to be submitted along with the Examination forms:

. 5 passport size coloured photographs (3cmx4cm) in school uniform (with

white background)
. 1 copy of the passport (First page). An undertaking from the
parents if
there is no Dassport or na
is diffe from or Birth Certificate - Page
showing the Parents'names and date of birth'
o Registration fee of Rp 5.OOO.OOO (Grade 10)
Registration fee of Rp.S.SOO.OOO (Grade L2l to be deposited in the Bank
central Asia (BCA) with A/c no. 1063001965 and the beneficiary is
Yayasan Gandhi semesta Prima (please utrite student's name, class
and uBoard Exam Fee' on Bank Deposit Slip)'
o Hard Copy/Soft Copy of bank receipt (to be submitted to the class
teacher)
o copy of Grade 10 Pass certificate (which has name and date of birth) - f'or
Grade 12 students.

The examination form (duly signed by parents) and fees should be submitted
on
or before 29tt'November 2O2L.

NOTE: In case there is a discrepaxcy 1n the name or date of birth of the

candidate, a form available for rectifying the sarne, must be taken from the
school
office.
