PERKHIDMATAN PERAWATAN DOMISILIARI (PPD) DI KESIHATAN PRIMER

Objective 1. Provide continuous health care to patients in need

2. Empower family members/caregivers in the aspect of patient health care at home,
8 health goals 1. Care provided at home or closer to home
of PPD 2. Seamless continuous care
3. Encourage self care
4. Empower caregiver
5. Reduce hospital congestion
6. Reduce complication arising from long term admission, reduce hosp cost
7. Promotes early healing in a home environment (person focused)
Team 1 MO, 1 Staff nurse and 1 MA
Criteria PPD  Pesakit warganegara Malaysia 
 Kes yang dirujuk daripada hospital kerajaan / klinik kesihatan 
 Pesakit mempunyai alamat rumah yang jelas
 Pesakit mempunyai penjaga yang sesuai dan bersetuju
 Pesakit tinggal dalam kawasan operasi klinik kesihatan
 Mendapat kebenaran bertulis dari penjaga
Referral 1. Hospital team identify case suitable for Domi care : MRS >4

2. Hospital/KK team will fill up PPD 001/2014 form, send the form and contact Penyelaras
PPD KK
3. Consent for from the family/care taker will be obtained by hospital team prior to
discharge
4. The aim/care planning of the PPD will be filled up by Hospital MO ; eg
i. Nursing care
ii. Medication/ depo
iii. Dressing
iv. Rt change
v. Cbd change
vi. Physio
vii. OT
viii. Kerja sosial perubatan
ix. Swallowing therapy
Meeting After received the referral, we will conduct the meeting with our DOMI team 1 day prior to
home visit
First visit Need to be seen and assessed by MO
Subsequent Done by MA/Staffnurse
visit Due to the pandemic and shortage of staff- Domi visit is being done once every fortnight
They will assess weather care taker able and confident to do the necessary tasks in term of
patient care
Discharge The period of service is for three (3) months
Patient will be discharged when the family member/guardian is able to take over the
patient's care
