AFFIDAVIT OF NEXT OF KIN

I, TIRSO ARIAS MACAWILE, of legal age, Filipino, widower, and with

residential address at Brgy. 2, Poblacion, Mercedes, Eastern Samar, after
having been duly sworn to in accordance with law, do hereby depose and
state that:

1. That I am the next of kin of Wenifreda E. Macawile, who died on

June 5, 2023;

2. That the decedent died without issue;

3. That no personal representative has been appointed for the

decedent’s estate in this state or elsewhere and no application for
such an appointment is pending in this state or elsewhere.

4. That this affidavit is made in support of the undersigned’s request

for the release of the Terminal Leave Benefits of the deceased
Wenifreda E. Macawile at the DAR Provincial Office of Eastern
Samar.

Further, your affiant sayeth naught.
