TERMS AND CONDITION:

1.    Each employee shall be given assigned targets ahead of time as bases for the performance evaluation for the Balance Score Card (“BSC”) Incentive Program;

2.    Work performance evaluation shall be based on the assigned targets provided to each employee;

3.    Evaluation shall be done by the employees’ respective department heads at the end of every month and the employees shall be rated in accordance with the Company’s work standards and the standards provided under the
Labor Code and relevant jurisprudence;

4.    The incentive amount shall be based on the score garnered by the employees pursuant to their assigned targets as shown in their respective BSCs;

5.    Incentives shall be paid every quarter starting April 2020. The amount of incentive shall vary depending on the Company’s collection for each quarter and shall be computed based on the employee’s rating multiplied by the
maximum incentive per employee for the quarter;

6.    Every after four (4) cycles of the BSC Incentive Program, the Company reserves the right to use the BSCs to determine the over-all performance of the employee in the Company. Employees determined to be not performing
based on the standards stated in item 3 shall be subjected to appropriate sanctions;

7.    The BSC and the evaluation shall be deemed confidential and shall form part of each employee’s 201 File.
The foregoing may be amended from time to time upon reasonable notice to the employees.
