Dear Sir / Madam,

I / We have applied a “Financial Facility” from Housing Development Finance Corporation Limited (“HDFC”). I/ We am aware, I/ We
may be eligible for subsidies / benefits under certain schemes launched by Government of India /State Governments/NHB/ or any
other scheme with Direct Benefit Transfer (“DBT”), in present/in future.

In connection with the same, I/We declare and confirm that:

- I/We hereby submit voluntarily at my/our own discretion, the physical copy of Aadhaar card/physical e -Aadhaar / masked
Aadhaar / offline electronic Aadhaar xml as issued by UIDAI (Aadhaar), to HDFC for the purpose of establishing my/our
identity / address proof.
- I/We voluntarily give my/our consent in connection with me/us availing the Financial Facility from HDFC and processing
instructions for the said purpose and hereby give consent to HDFC for verification of my/o ur Aadhaar to establish its
genuineness through Quick Response (QR) code embedded in the Aadhaar card or through such other acceptable manner
stipulated by UIDAI or in terms of any applicable law for the same.
- The consent and purpose of collecting Aadhaar has been explained to me/us in local language. HDFC has informed me/us
that my/our Aadhaar submitted to HDFC herewith shall not be used for any purpose other than mentioned above, or as per
requirements of law. HDFC has informed me/us that this consent and my/our Aadhar will be stored with HDFC.
- The annual household income of our family is Rs ____________.

I/We hereby declare that all the information voluntarily furnished by me/us is true, correct and complete. I/We will not hold HDFC
or any of its officials responsible in case of any incorrect information provided by me/us.
