AFFIDAVIT
(RE: Sworn Statement)

I, ERMA G. WENCESLAO, of legal age, Filipino, single and with residence

and postal address at Del Pilar Gomez St., Brgy. Zone 3, Digos City, Davao del Sur, after
having been sworn to in accordance with law, do hereby depose and state:

1. That I am the same and identical person who caused the execution of this
instrument;

2. That I am applying for any position available in any court of the 11 th Judicial
Region;

3. That I have no relatives within Third(3 rd) Degree either of Consanguinity or

Affinity who is an incumbent employee in any branch of Court of the 11 th
Judicial Region;

4. Should the Supreme Court of the Philippines ascertain that I have relatives in
any Court of the 11th Judicial Region where I will be applying, the Supreme
Court reserves the right to cancel the processing of my application;

5. That I understand that should I be employed, any misrepresentation on that

matter is a ground for administrative case and termination;

6. That I am executing this affidavit in order to attest the truth and veracity of the
foregoing facts and for whatever legal purpose this may serve best.
