AFFIDAVIT OF RECOVERY OF LOST TITLE

I, MARCELINA S. MARTIR, of legal age, Filipino and a resident of Barangay

Dadiangas Heights, General Santos City, Philippines, after being duly sworn in accordance with
law, do hereby depose and state that:

1. I am the Vendee of a parcel of land registered in the name of Bessie T. Yap,

covered by Transfer Certificate of Title No. T-77963 and more particularly
described as follows;

“A parcel of land known as Lot 18-F-3-B-16, Psd-11-016566, being a

portion of Lot 18-F-3-B, Psd-11-012997, situated in Barrio of Siguel,
General Santos City, containing an area of Five Thousand Four Hundred
Forty Eight (5448), square meters, more or less.”

2. A certain Amelita L. Tupas, executed an Affidavit of Loss pertaining to TCT

No. T-77963 on September 4, 2020 before the office of Atty Rommel L.
Cahayag, known as Doc No. 429, Page No, 86, Book No, 107, Series of 2020.

3. On September 8, 2020 the said Affidavit of Loss was registered and annotated at
the back of the Transfer Certificate of Title No. T-77963, before the ffice of
Registry of Deeds, General Santos City known as Entry No. 2020004205.

4. I hereby declare and confirm that the said Transfer Certificate of Title No. T-
77963 is still in my custody and had never been loss.

5. The purpose of this affidavit is to prove that the aforementioned lost title was
never lost and the person who executed is unknown to us or to the original
registered owner.

6. There is a cogent need to inform the Registry of Deeds of such facts and as basis
to cancel the aforesaid annotation of the Affidavit of loss.

7. I am voluntarily executing this affidavit to attest to the foregoing facts of loss

and for whatever legal purposes this may serve best.
