REQUEST FOR RECONSIDERATION

Dear MS. ____________

This has reference to the notice of post-disqualification dated February 3, 2022,

notifying the undersigned company of the disqualification in the public bidding
for the _____________________________________________for failure to
submit the documentary requirements/conditions required under Section 34 of
the 2016 Revised Implementing Rules and Regulations of RA 9184.

I am writing to request for reconsideration said disqualification. Said failure to

attach the documentary requirements was a mere oversight due to the
voluminous contents of the documents which the undersigned submitted.

Attached herewith are the following documentary requirements/conditions

supporting my request and in compliance with Section 34 of the 2016 Revised
Implementing Rules and Regulations of RA 9184:

a. Signed list of key personnel

b. Certificate of Employment and other documentary requirements for the
Key Personnel (Electrician to be assigned to the contract)
c. Amended list of equipment s
d. Notice to Proceed for FMRs stated under the statement of all ongoing
Government and Provite
