Mother’s Name; Category; Contact Details-like Residential Address & Phone/Mobile Number, e-mail
ID etc…, e-request for correction(s) must be generated, followed by submission of self-attested
copy(ies) of documentary evidence(s) (like School Leaving Certificate, Photo ID etc…) at the
Students’ Facilitation Centre, University Head Office. Your e-request shall be processed only after
submission of necessary documentary evidence(s) at SFC.
CORRECTION(S), IF ANY, THE e-REQUEST FOR THE SAME SHOULD BE GENERATED BY THE
CONCERNED STUDENT WITHIN 10 DAYS AFTER END OF EXAMINATION.

Instructions:Note: Admit Card is valid if contains the signature of Deputy Registrar (Exams). Student must preserve & produce this at each session of the exams
without which Entry into the Exam Hall may be disallowed. Adoption of recent changes in the time-table is responsibility of the Student.
