AA) This checklist shall be filled up prior commencing any painting job inside an enclosed compartment e.g. ballast tanks,
FW tanks, holds, etc. Separate checklist is to be filled for each compartment to be worked in.

1.1 Has the "Enclosed Space Entry" checklist been filled ?
1.2 Has the spray painting equipment been checked and confirmed that all in order ?
1.3 Have arrangements been made for the space to be continuously ventilated throughout the period
of occupation and during work breaks ?
1.4 Is it confirmed that there will be no interruptions in the ventilation of the compartment ?
1.5 Has the following equipment been checked and found suitable for use in explosive atmospheres :
a) Lighting inside the compartment
b) Fans and blowers used for ventilation
c) Portable radios / Walkie-talkies used for communication
1.6 Are safety data sheets (MSDS) of the paint and solvent/thinner available, read and understood by
all concerned ?
1.7 Have the crew and other persons assigned the job been made aware that excessive thinners
should not be used as it will increase the risk of forming a highly explosive atmosphere inside
the enclosed compartment ?
1.8 Have arrangements been made for regular atmosphere checks to be made (every 2 hours) while
the space is occupied and after work breaks?

1.13 Have you received permission from the Head Office to commence spray painting inside the
enclosed compartment ?

2.1 SECTION 1 of this permit has been completed fully ?
2.2 I am aware that the space must be vacated immediately in the event of ventilation failure or if
atmosphere tests change from agreed safe criteria.

Notes: In order to obtain a representative cross-section of the compartment's atmosphere, samples should be taken from all areas / parts and through as many openings
as is possible. Ventilation should be stopped for about 10 minutes before the pre-entry atmosphere tests are taken.
