MT: Reason for Entry:
A. 'Risk Assessment' will be carried out by the Master, who will evaluate the hazards and identify mitigating measures.
B. Completed 'Risk Assessment' form should not be construed as permission for man entry. A separate permit (S‐6CT.29b C) shall be filled in by the
'Designated Officer' after carrying out necessary safety checks and shall be issued within 12 hours of completing this form.
C. No more than 2 tanks to be entered at the same time.

Note: If cargo tanks are not free from hazardous cargo residues, further cleaning/ ventillation shall be carried out to remove residues from the tank and till
