Welcome to THE PARK by CBD! You and your dog(s) and any guest(s) with you, are to
use the facility and avail of our services at your own risk. You are solely responsible for
supervising the dogs you bring while using the dog park and following its pertinent rules and
regulations and deciding if this is an appropriate activity space for you and your dog(s).

While we have strict rules we adhere to for the use of our facilities and availing of our
services offered, understandably so, not all hazards associated with dog activities can be
foreseen. There are certain risks inherent to dog(s) and/or outdoor activities. It is impossible to
guarantee absolute safety while using the dog park.

WAIVER AND RELEASE

Please read this form and be aware that, to use this facility (facility includes the dog
park and surrounding areas) and to avail of our services (training and boarding), you assume
the full risk and legal liability for all actions and damages from the actions of your dog
and yourself. You waive and release all claims for injuries, damages, or loss you, your
dog, or someone accompanying you might sustain as a result of using this facility.

By confirming this waiver, you recognize and acknowledge that using this facility
entails risks of damage, loss or injury to animals, persons and/or property (including
yourself, other persons, your dog and other dogs or other animals) and you agree to
assume the risk and responsibility for any injuries, damages, loss, liability, costs and
expenses, that;

1. You, or your dog, or any person accompanying you might sustain or incur
in this facility or;
2. any other person or animal might sustain as a result of you or your dog’s actions
at this facility.

By signing this form, you agree to waive;

1. The Park by CBD, and its management;

2. Judi K9 Training Services and its team and;
3. any trainer, helper, employee/volunteer

from all damages, injury, loss, liability, or expenses you may now have or which may
accrue to you and any other person claiming through you or on your behalf associated with this
facility.

By signing this form. You agree to indemnify;

1. The Park by CBD, its owners, and management team;

2. Judi K9 Training Services management and its team and;
 3. any trainer, helper, employee/volunteer

from losses, claims, damages, liabilities, cause of actions, and expenses occurring from
your use of the facility; including any losses, claims, damages, liabilities, cause of actions and
expenses for personal injury to or death of any person or animal, or damages to property.

Each dog owner and/or handler must sign this Waiver and Release prior to any use of
The Park’s facilities and availing of any of the services being offered. If more than one person
from your household brings your dog(s) to the facility, each person must sign this Waiver and
Release.

Any person under the age of 18 requires the confirmation and/or supervision of a parent
or guardian.

I HAVE READ AND UNDERSTAND THE ABOVE INFORMATION, RISK AND WAIVER AND
RELEASE OF ALL CLAIMS. I AGREE TO ABIDE BY ALL RULES OF THE PARK BY CBD AND
RELATED FACILITIES.
