1. What strategies would you employ to obtain a complete health history?

Numerous strategies can be used to obtain a history from patients who cannot directly

communicate. The information about the health history of infants can be obtained by

asking the family members. Additionally, the body of the patient can communicate to the

nurse, for example, changes in body condition such as discolouration and temperature

changes. Crying and whimpering can also obtain patient information (Flungelman, 2021).

Tools such as the NIPPS pain scale can determine patient discomfort levels based on the

assessment data for many grown patients who cannot communicate. Some medical

translators are useful for collecting patient information for patients who speak foreign

languages, especially when they speak a foreign language. The elderly and mentally

challenged can be approached slowly and simply to obtain patient information.

Additionally, using electronic health records to access patient information could provide

patient history (Flungelman, 2021).

2. Provide a rationale for why you think these strategies would be effective.

The proposed strategies would be effective because they address the individual

communication needs of the patients. For example, infants cannot communicate. Hence,

the family can provide essential information needed to treat the infant. Others who cannot

verbally communicate can respond with their bodies to give nurses the necessary patient

information (Flungelman, 2021). For example, nurses could determine if an individual is

ill by taking the body temperature and observing unusual changes to the skin colour and

tone. Medical technology, such as translators, is particularly effective for foreign patients.

Cognitive impairment leads to delayed responses, so approaching such patients slowly


facilitates healthcare professionals to obtain essential information. Electronic medical

records are also reliable for giving patient history and information.
