MEMORANDUM

Re: Equipment shortages

This week our emergency order of additional sterile gloves and PPE arrived considerably short of the required number of
units causing us to have to stretch our existing supplies and reserves thinner than they already were.

The supplier - Alder Medical Supplies - has been contacted and notified of this issue and has dispatched the required supplies
which are currently in-transit to the hospital.

The cause remains undetermined at this time - however, we had been able to ascertain that the supplier received only the
regular order from our account and they were never made aware of the additional units we ordered. As far as we can tell
there is no record in their system despite our hospital updating the order 1 week prior to the shipment date.

At the time of writing the cause is assumed to lie in the new ERP V software, Holistic Software Solutions Five (HSS-V), which
manages the shipping and receiving system in conjunction with Alder Medical Supplies. As you know, we are currently in
phase 1 of the rollout and want to figure this out before using know software with more critical supplies and hospital systems.

We have convened an internal committee to help resolve this matter, and hired an outside consultant from Aria consulting - a
local firm which specialises in enterprise software integrations and issues.

I will be working directly with the independant consultant to arrive at a swift resolution to this issue as well as develop a
strategy to prevent similar issues in future.

Updates will be sent as relevant information comes to light.
