Histological examination

Macroscopical findings:
Six samples ranging from 0.2 x 0.2 x 0.2 cm and 0.8 x 0.6 x 0.5 cm were received and processed.
All samples were examined histologically as is. Location provided in submission form: Footpad

Microscopic examination:
The following staining was performed according to standard operation procedures: H&E.
Skin. The epidermis is diffusely & irregularly thickened/ hyperplastic with numerous, rete ridges infiltrating into
the underlying dermis. Overlying the intact epidermis there is a thick layer of orthokeratotic hypereosinophilic,
compact and basket-weave hyperkeratosis.
Within the superficial dermis, within the interstitium, surrounding the vessels and infiltrating into the deeper
layers of the dermis, there are coalescing foci of inflammation characterized by mainly neutrophils as well as
fewer histiocytes, lymphocytes and plasma cells. The interstitium is moderately distended by increased amount
of collagen (fibrosis).
Within the dermis, there is a locally extensive area of granulation tissue. This is characterized by a proliferation
of fusiform, plump cells (fibroblasts) within an edematous stroma with collagen deposition and numerous,
variably sized, congested vessels perpendicularly oriented and lined by a plump, reactive endothelium. The
vessels are diffusely congested lined by a reactive endothelium. The lymphatics are diffusely and moderately
ectatic.

Skin. Dermatitis, suppurative, lymphohistiocytic, interstitial and perivascular, with fibrosis and granulation
tissue

Critical report:
A pododermatitis is a very heterogenetic disease, that can be induced by a multiplicity of causes. Besides
foreign bodies and trauma, a multitude of chemical and mechanical irritations as well as allergies are possible.
Parasitic, fungal and bacterial infections can occur. Psychogenic dermatitis due to licking is possible as well. In
many cases, the primary cause can not be detected and chronic, difficult to treat, bacterial infections develop.
The presence of neutrophils within the sections examined may suggest an infectious, most likely bacterial,
component. This could be either primary or secondary, thus complicating the clinical picture. The amount of
granulation tissue and fibrosis suggest a chronic-ongoing process.
In this case, performing further ancillary testing, such as bacterial culture may be beneficial to potentially isolate
a causative agent.
