Greetings of Peace!

I am Myka Pearl J. Daniel, a G12-S5 student. I am writing to your office to request to

kindly allow me to submit my output in the activity in FILI 300 of Ms. Jessah Romero.

During the time of submission, a close loved one of mine passed away. Three (3) days
after his death, he was buried. I got to see him once on the day of his burial. I was still
unexpectedly and profoundly affected by this loss.

My mind and body were not exactly in the best condition as I was unable to properly
execute some of my deliverables in both extra- (Madayaw 5.0, Debate, EXIMIUS 22, etc.) and
co-curricular (PTs, Quizzes, etc.) activities and responsibilities which include my task in Ms.
Jessah’s subject. I overlooked if the output was truly passed or not.

With this, my parents can attest to my situation with sincerity. Also, attached herewith is
proof that my loved one has passed away.
