    This form must be provided upon application. This letter must include the
    Applicant's Full Name, Name of School, Grade Level, and is to be
    acknowledged and signed by your school's person in-charge.

    TO THE PERSON IN-CHARGE:

    By signing this letter, you ACKNOWLEDGE and CERTIFY that the applicant
    below is compliant to the following qualifications:

    A . Must be A BONAFIDE STUDENT in Western Visayas for S.Y. 2022-2023

    B . Must NOT HAVE had any failing grade during S.Y. 2022-2023

    C . Must NOT HAVE been SUSPENDED and must NOT BE under disciplinary
    probation during S.Y. 2022-2023; and

    D . Must be of GOOD MORAL CHARACTER

    TO THE PERSON IN-CHARGE:

    I, (Person in-Charge), acknowledges and certifies the application of (Name of


    Applicant), a (Grade Level) Student of (School Name), and is compliant to the
    qualifications set for the said position. Moreso, by signing this letter, I am also
    the school's contact person for all engagements with The Peer Project.
