CERTIFICATION OF COMPLETION OF REPAIRS

I hereby certify that all necessary repairs in connection with damage sustained to the
property (AKIS BARBERSHOP) owned by John Benederio located at Brgy. Pob Ilaud Zarraga
Iloilo resulting from VEHICULAR ACCIDENT on SEPTEMBER 4 2022 have been completed in
a satisfactory manner and that no liens have been or will be filed against the property as a result
of labor performed or material used. That the contractor will not held liable for any future
deficiency in the construction.

The above referenced property has now been restored to the condition existing prior to
the date of damage, on this date September 8, 2022.
