SWORN AFFIDAVIT

I, JOHN KEVIN R. VILLAREAL, of legal age, Filipino, married, and with residential
address at Barangay Angeles, Atimonan, Quezon after being duly sworn according to
law, hereby depose and state that:

1. I am the buyer of that portion of parcel of land with a total area of 10,340
square meters more or less covered by Transfer Certificate of Title No. 066-2013002350
of the Registry of Deeds for Quezon Province to the extent of 8,048 square meters only
pursuant to the Kasulatan ng Bilihang Lampasan at Patuloyan dated March 5, 2020;

2. I am currently in the process of registration of ownership over that

portion of the parcel of land subject of the sale to the extent of 8,048 square meters;

3. In view of the foregoing, I hereby request for the segregation of

ownership of the said parcel of land under the following:

the said land in the name of above-stated owners pursuant to the sale thereof.
