Dear Madam:

Greetings of peace and goodwill!

I have the honor to inform your good office that I would like to apply for transfer of my teaching
assignment from my present post, Melgar National High School to any secondary school at Baco near to
my residence due to the following reasons:

a. that I am a legitimate resident of Brgy. Sta. Rosa I, Baco, Oriental Mindoro and had my practice
teaching at Baco National High School;
b. that I had been a secondary English teacher applicant of Baco last 2015-2016;
c. that I can save much money from house rental and time from traveling if I will be assigned to
school near my residence;
d. that I suffer from severe pain due to dysmenorrhea every month making it hard for me to live
independently;
e. that I want to serve my fellow Bacoeños effectively and efficiently as a teacher.

I would sincerely appreciate the feedback from your good office regarding this matter and I am
willing to accomplish the requirements needed for the formalization of the whole process. I am hoping
for your favorable response and kind approval.
