AFFIDAVIT OF LOSS

I, ERUEL DARWIN ALON RICO, of legal age, Filipino Citizen, married, and a
resident of Lopez Compound E. Sabellano St., Pardo, Cebu City, Philippines, after
having been duly sworn to in accordance with the law, hereby depose and state:

1) That I am the rightful owner of a parcel of land known as Lot 103, Block 12, of
the consolidation-subdivision plan, P-07-003964 with an area of 150 square
meters situated at Brgy. Candulawan and Lagtang, Talisay City, Cebu,
Philippines, and covered by Transfer Certificate of Title No. 102-2013005751

2) That the said lot is covered by Transfer Certificate of Title No. 102-
2013005751 is registered in the name Sps. Romelito D. Hiramis and Rowena R.
Hiramis and I acquired the said lot through sale. They executed a Deed of
Absolute Sale on the property in my favor. A copy of the Deed of Sale is
hereby attached as Annex “A”;

3) That through inadvertence, I unfortunately loss possession of the owner’s copy

of Transfer Certificate of Title No. 102-2013005751 when I processed the
payment of the Capital Gains Tax and Documentary Stamp Tax at the Bureau
of Internal Revenue after the execution of the Deed of Sale;

4) That I could not recall where I left the envelope where the said owners copy of
Transfer Certificate of Title No. 102-2013005751 was place. I could only
surmise that I left it either in the bank, in the BIR or in Robinsons Department
Store in Lawa-an, Talisay City, Cebu;

5) That when I went to those places where I can possibly retrieve the said owner’s
copy of Transfer Certificate of Title No. 102-2013005751 I could not
anymore locate the same;

6) That I believed that the owner’s copy was lost and that despite diligent efforts
exerted to search for the said owner’s copy or duplicate of Transfer
Certificate of Title No. 102-2013005751, the same can no longer be found
such now that I believed that it is now lost beyond recovery;

As such, I am executing this affidavit to attest to the truth of the foregoing facts
and for the purpose of annotating the same on the original Certificate of Title on records
at Register of Deeds of Cebu Province.
