AFFIDAVIT OF LOSS

I, HYACINTH ROSE G. GONZAGA, Filipino, of legal age, married and a

resident of Brgy. Zone3, Canton Village, San Isidro, Talisay City, Cebu Philippines after
having duly sworn to in accordance with law, hereby depose and say:

1. That my father, NESTOR D. GONZAGA, is the legal holder of a

SHOTGUN with Serial No. 1038889 and a REVOLVER with Serieal No.
AB-98829;

2. That my father has already migrated in the United States of America and
comes very often to Philippines for a visit;

3. That before my father left the Philippines, he entrusted the SHOTGUN

with Serial No. 1038889 and a REVOLVER with Serieal No. AB-98829 to
my brother in the person of Clint Mathew Gonzaga;

4. That the said SHOTGUN with Serial No. 1038889 and a REVOLVER with
Serial No. AB-98829 was inadvertently lost in the possession of my brother
when it could not be located in the cabinet in our house where he placed the
said firearms;

5. That my brother Clint Mathew Gonzaga died last year and after his death, I
made a search of the said firearms;

6. That despite diligent search and efforts to look for the said SHOTGUN
with Serial No. 1038889 and a REVOLVER with Serieal No. AB-98829, I
could not find the same such now that I believe that it is now beyond
recovery;

7. As such, I am executing this Affidavit of Loss to attest to the truth of the

foregoing and for whatever purpose that this affidavit may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 22 nd day of
