Retrieval of Separated Instruments Using a Combined Technique


of Ultrasonic and Microtube Methods.

Objectives: To describe the clinical scenario of a separated intracanal instrument


removal by means of a combined technique of an ultrasonic method and a microtube.
Methods: A 36-year-old female patient was referred for removal of a separated
instrument in the right maxillary first molar. The intra-oral periapical radiograph
revealed separated long instrument fragment within almost the entire length of the
mesio-buccal canal.
The removal of the fragment was attempted with the endorescue kit but without
success, as a sufficient grip of the fragment could not be established. The instrument
was locked within the dentinal wall of the root canal.
Ultrasonic tips were used to remove dentine surrounding the coronal end of the
separated fragment. Then the microtube method was used to retrieve the instrument
fragment using Micro-tip (ultradent) and K file (size 20).
Results: Removal of fractured instruments is one of the most difficult operative
procedures in endodontics. Although a number of different devices and techniques
have been developed, none are successful in all cases.
Conclusions: this system renders a safe, inexpensive, and predictable method of
retrieving separated tips from root canals. It is safe in regards to amount of removed
dentin.
