SUBJECT : REQUEST FOR CCTV FOOTAGE

Dear Dr. Gonzales,

We would like to respectfully request a copy of the CCTV footage placed on the post
northeast of Bai Ma Jiang KTV Bar along Roxas Blvd. Brgy. Tambo, Parañaque City at
approximately 8:40 AM on April 4, 2022. This request is respectfully being made for
legal purposes in relation to criminal case no. XV-12-INQ22D0381. This case involves a
shooting incident between one of our employees, Mr. Renie Edison Donato y Dela Vega,
who was working as a security guard for Bai Mai Jiang Hi Song Corporation. The
specific purpose of this request is to properly ascertain what happened during the
shooting incident between Mr. Donato and an unidentified robber.

Acquisition of the CCTV footage would greatly aid the ongoing case regarding the
incident.
